WeCare Companion is an AI-enabled chronic care platform designed to support CKD and dialysis patients managing complex daily routines. The product reimagines how therapies, medications, vitals, and care communication come together in one calm, clinically safe experience, while laying a scalable foundation for AI-driven insights, adherence support, and remote care coordination.

Solution
The solution was a user-centered design of WeCare Companion, built from the ground up around real patient routines and clinical workflows. We translated complex therapies and daily care tasks into guided, step-by-step flows, unified device data and manual logs into clear, readable views, and established consistent design patterns to support accuracy and scale. This created a stronger product foundation, a calmer and more trustworthy experience for patients, and a solid base for expanding AI-driven insights, adherence support, and clinician visibility.
UX Strategy Roadmap
This UX strategy roadmap shows how I moved WeCare Companion from clinical insight to shipped outcomes while maintaining alignment across product, engineering, and care teams. The work started with research, combining patient input, clinical reviews, and UX audits to identify friction in daily care routines, data clarity, and clinician visibility.
From there, I mapped the information architecture and core patient journeys, validating structure and flows through wireframes and usability checks before investing in visual polish. Once the foundation was solid, I guided UI design and implementation with a focus on clear handoff, accessibility, and clinical safeguards, then closed the loop post-launch by feeding real-world usage and feedback back into the next iteration cycle.
